Will I receive written confirmation that my child is enrolled? |
Yes, you will receive a confirmation sent to the email address listed on your application (preferably a parent/guardian email). Please be sure to add (JavaScript must be enabled to view this email address) to your list of qualified senders in your address book to avoid missing any important details. Enrollment will only be secured when both the completed application and the $200 non refundable deposit are received. Please read your confirmation thoroughly as not to miss important information. |

What type of supervision do you provide? |
Campers are supervised by staff members during the entire camp (on and off the field). No one is allowed to leave campus. Boys and girls stay on separate floors and are not allowed on the others floor. There are camp counselors on each floor. |

What are the housing accommodations? |
Camper will be staying in dorm-style accommodations- and each room can accommodate two campers. You may request whom you would like to room with, and mutual requests are honored. If no request has been made, a roommate will be assigned to you based on your age. Special requests for triples are taken into consideration, these are extremely limited and not guaranteed. Rooming assignments are made one week prior to each camp session, changes made after that time cannot be guaranteed. We do our best to honor all requests. Be sure to bring single bed sheets, pillow, blanket, fan, and lamp. There are pay phones on each floor (bring a phone card or buy one in our camp store) or campers are welcome to bring a cell phone (to use during free time only). There are also computers available for campers to check their emails during their free time. |

Are all the meals provided? |
Yes, all meals are provided in the cost of camp. Please remember that the first meal served will be dinner on Sunday. Campers eat in an on-campus cafeteria offering a variety of selections and unlimited portions. Lunchtime always offers a hot meal/salad bar/sandwich bar. Dinner always offers a hot meal/salad bar/pasta bar. A snack bar is available each night to provide snacks and drinks after the evening games. Campers may also order pizza for the evening activities. |

Does my child need any spending money? |
At check-in time, each camper will be required to set up a camp bank. (Suggested $75) The staff will keep track of the money spent by each camper which can be used to purchase snacks, order pizza and purchase items at the Soccer Extreme Camp Retail store. This money will also be used as a security deposit for the rooms in case of damage. Any money left over will be sent back in the form of a check in the two weeks following the end of your camp week. |

Where are the residential camps located for 2010? |

What about Full/Extended Day Campers within our residential locations? |
Extended Day Campers can arrive as early as 9am and stay for the full day of training and scrimmage till 8:30pm.
Both meals are included, so all you need to pack is a water bottle, cleats, shin guards, sunscreen, bathing suit and
towel (optional swim time), and an extra change of clothes if you choose to stay for the evening activities.
It is recommended that you stay through the nightly scrimmage, which is from 7:00 pm- 8:30pm. (Parent's are welcome to watch!).
You can choose to leave after the scrimmage or stay for the evening activity and be picked up no later than 10 pm.
The evening activities include a talent show, karaoke, DJ, lectures, films. (age appropriate) Check-In Time is Sunday at 1pm for day campers.
The first session will follow.

What does my child need to pack for camp? |
It is important that you bring the following items with you to camp:
lamp and fan; soccer equipment: cleats, turfs (which are indoor/outdoor cleats-
if you don't have any- sneakers will be fine), playing shorts, t-shirts, shin guards,
socks (prefer dark colors); equipment bag; water bottle, sunscreen; bed linens: sheets, blanket,
and pillow; towels for showering, sandals; alarm clock; leisure clothing for night time activities: i.e. films, lecture, and dance.
Not required, but recommended: props for talent show; radio and music, playing cards, books, etc.
Please call ahead should you need to request special soccer equipment brought to the camp. |

What about Check-In/Check-out Times? |
For residential camps, Check-in time is between 12-1pm. Lunch will not be provided on the day of check-in. The first meal served is dinner on Sunday. Check-out time is Thursday* at 2:30 p.m. There will be a final tournament, which begins at 3:00pm. Parents are welcome to come watch.

Does my child need a Medical Form? |
A medical form/waiver is essential for your child to attend camp.
This must be sent in no later than June 1st. If your child's doctor's appointment is
scheduled for a date later than this, please fax the form in as soon as it is filled out to 860-355-1288.
Please be aware that the form must be filled out completely in order for your child to participate in camps.
If your child has a medical form from school or another camp- you can attach this to our form- but be sure
to complete the top portion, the insurance portion, and the waiver on the bottom of our form. |

What is your Tax Id Number? |

What is Soccer Extremes Refund Policy Should we choose not to attend? |
For all residential camps: The deposit of $200 is non-refundable. Cancellations made 15 days prior to the start of camp are subject to a 50% refund and also 50% credit to attend Soccer Extreme Camps in 2011, less the cost of the $200 nonrefundable deposit. If a student leaves camp early for medical reasons, the portion of time missed from the camp less the deposit can be used as credit to attend Soccer Extreme Camps in 2011. We reserve the right to handle all refunds on an individual basis.
Final balance for all camps is due no later than June 1, 2010. You may lose your reservation in the camp if the final payment is not received by this time.
For all day camps: A $25 management fee will be incurred.
